Effective January 31, 2008 the Human Resources Department of the former Pascack Valley Hospital will officially close. This website will be maintained for two years from the closing of the hospital.
Personnel Files
Iron Mountain Company was retained to store hospital personnel files and employee health records. Iron Mountain can be reached at (845) 338-2993. Their website is www.ironmountain.com. Records will be available by the end of March, 2008.
Employment Verification
Employees who were laid off due to the closing of the hospital were issued a letter of employment on their last day. This letter should be used when verification of employment is needed in the future. Verification of employment for those employees who left prior to the closing of the hospital can be obtained by contacting Iron Mountain. Again, their number is (845) 338-2993.
Pension & Profit Sharing Plans
The Pascack Valley Hospital Pension Plan is insured by the Pension Benefit Guaranty Corporation ("PBGC"), a federal agency. Pascack Valley Hospital Association, Inc., the contributing sponsor of the Pension Plan, is seeking a distress termination of the Plan. Until the Pension Plan is terminated, Buck Consultants will administer it and answer any of your questions at (866) 283-7209. You will be notified when the Pension Plan is terminated. When the Pension Plan is terminated, PBGC will be the statutory trustee of the Plan and will pay your benefits, up to the statutory limits.
The Lifekey Ventures Profit Sharing Plan has beeen terminated as of December 31, 2007 Each participant will receive a distribution of the balance of his or her account. If you are a participant in the Lifekey Profit Sharing Plan, you should have received your distribution election forms before March 1, 2008. If you have not received you distribution election forms, please call Buck Consultants at (866) 283-7209.
COBRA & other benefits
If you have questions regarding COBRA benefits, please contact Linda Suarez of the New Jersey Hospital Association at (609) 275-4064. If you have questions regarding medical, prescription, or dental benefits, please contact your insurance carrier first. The telephone number is on the back of your insurance card. 403b Plan
If you are inquiring as to how you can rollover your 403B, please contact Met Life at (800) 560-5001. You will need to know the plan number which is 1071085.
W-2s
The 2007 w-2s were mailed out January 21, 2008 by the Payroll Department. Any questions about your w-2 should be addressed with Payroll at (201) 358-3043. Please note that the 2008 w-2s will be mailed out by April 2008. Please be sure to retain these forms in a secure place for your tax filing in 2009 as copies will not be available.
$10,950 pay out
If you are calling regarding the $10,950 Pay-out Summary, this form was supplied to you to explain the payment that you received. It also serves the purpose of showing you the additional amount you would have been paid if not for the U.S. Bankruptcy code limitations on salary and accrual time pay out. The bankruptcy court will contact you with instructions on how to file as an unsecured creditor for the amount listed at the bottom of the $10,950 Pay-out Summary sheet.
