RECORD RETENTION PLAN
IMPORTANT PUBLIC NOTICE
In accordance with The State of New Jersey’s Division of Archives and Records Management guidelines (NJDARM), Pascack Valley Hospital (PVH) has made arrangements for its patient and business records to be available and accessible upon request.
It should be noted that there is no single long term storage company capable of managing all of PVH’s hard copy and electronic records. Given the need for specialized software, hardware and IT specialists, several remote hosting companies have been contracted to archive electronic records. Iron Mountain will store all paper documents and radiographic films beginning mid March, 2008.
A complete listing of the companies is attached. PVH will be running weekly public notices in local newspapers for a three-month period informing patients of all these arrangements. Additionally, all of this information will be on the hospital’s website.
It is anticipated the transfer of records to these companies will be completed by the end of March, 2008 except for radiology x-ray films taken after September, 2005 and laboratory tests taken prior to June, 2006. PLEASE NOTE: THESE RADIOLOGY AND LABORATORY RECORDS WILL BE AVAILABLE BEGINNING MAY 31, 2008. A copy of the hospital’s detailed Record Retention Plan is on file with the State of New Jersey’s Department of Health & Human Services.
Please be advised that in accordance with state regulations, Pascack Valley Hospital (PVH) has arranged for long term storage of its patient and business records with the six storage companies listed below. It is anticipated that the majority of records will be available at these locations by the end of March, 2008. Please allow 2-3 weeks to process all requests. Release of records will be expeditiously processed for a medical emergency upon physician request.
